Release of Information Desk Closed
Health Information Management (HIM) has closed the Release of Information desk at the Acequia Wing. All patient release of information requests can be submitted via email, fax, or mail.
We understand that access to your medical information is important and essential; we will still be available to process your requests for release of information remotely. We will do our due diligence to process your requests in a timely manner.
Patient Requests for Medical Records/Results
Once Kaweah Health's Health Information Department has received your request, it can take anywhere from 3 - 5 days to process it. To request a copy of your medical record, please complete the “Authorization to Disclose Health Information” form below:

Medical Provider Requests for Medical Records/Results
For continuity of care purposes, please fax or email your request on your letter head and include the following information on your request:
- Patient Name
- Date of Birth
- Dates of service being requested
- Type of reports being requested
