Preceptors

Cory Nelson, PharmD, BCACP

Dr. Nelson is the Outpatient Pharmacy Coordinator and the Residency Program Director for the PGY2 Ambulatory Care Pharmacy Residency. In this role he coordinates the ambulatory care service lines and assists with other outpatient areas. He practices at the district's rural health clinics and collaborates with other medical providers in the area. He received his Doctor of Pharmacy from Idaho State University College of Pharmacy in 2014 and completed the two-year Pharmaceutical Care Leadership Residency with the University of Minnesota in 2016. Clint Brown, PharmD

Clint Brown, PharmD, is the Outpatient Pharmacy Manager for Kaweah Health. As such, he oversees all outpatient aspects of the pharmacy department including the ambulatory care team, a home infusion pharmacy, and two retail pharmacies. Prior to this he was an Oncology and Pain Management Clinical Pharmacist at Kaweah Health for 8 years. Dr. Brown completed his PGY1 pharmacy residency in 2009 with the Veterans Affairs Ambulatory Clinic in Pocatello, Idaho. He received his doctor of pharmacy degree from Idaho State University in 2008. In 2014 Dr. Brown and the Pharmacy Pain Management Service were recipients of the Best Practice Award from the American Society of Health-System Pharmacists (ASHP) for their work with implementing a pharmacy driven pain management service in the inpatient setting and also received recognition from the California Society of Health-System Pharmacists (CSHP) highlighting this service as an Innovative Pharmacy Practice. Kelly Mendoza, MS, PharmD, BCPS

Kelly Mendoza, MS, PharmD, BCPS is a Pain Management Clinical Pharmacist at Kaweah Health (Kaweah Health). She practices in ambulatory care as an advanced practice provider in chronic pain management at Kaweah Health’s Kaweah Health Specialty Clinic Center and Lindsay Rural Health Clinic. On the inpatient side, Dr. Mendoza serves as an opioid stewardship pharmacist and also practices acute pain management as a member of the ASHP Best Practice Award-winning (2014) Pharmacy Pain Management Service. Dr. Mendoza is a board-certified pharmacotherapy specialist and serves as a preceptor for APPE students and PGY1/PGY2 Ambulatory Care residents. Dr. Mendoza started at Kaweah Health in August 2017 after completing her PGY2 Pain Management and Palliative Care pharmacy residency at The University of Maryland School of Pharmacy in Baltimore, MD. Dr. Mendoza received her Doctor of Pharmacy degree in 2015 from Midwestern University, College of Pharmacy-Glendale in Glendale, AZ and then completed her PGY1 Pharmacy Residency at Banner Estrella Medical Center in Phoenix, AZ. Currently, Dr. Mendoza is an active member of the American Society of Health-System Pharmacists, Society of Pain and Palliative Care Pharmacists, and Phi Health Chi and serves on the recruitment committees for both Kaweah Health’s PGY1 and PGY2 Ambulatory Care pharmacy residencies. April Huang, PharmD, BCACP

Dr. Huang is an ambulatory care clinical pharmacist at Kaweah Health. She practices at the Kaweah Health Specialty Clinic Center, with a focus on employee wellness initiatives. Dr. Huang serves as a preceptor for the PGY2 Ambulatory Care Residency Program, as well as a preceptor for APPE students. She received her Doctor of Pharmacy from the University at Buffalo School of Pharmacy and Pharmaceutical Sciences in 2014. Dr. Huang completed a PGY1 Community Pharmacy Residency with Azalea Health in Palatka, Florida in 2015, followed by a PGY2 Ambulatory Care Residency with the Saint Louis College of Pharmacy and the Saint Louis County Department of Public Health in Missouri in 2016. Kelly Banker, PharmD, BCPS,BCPP

Dr. Kelly Banker is an Ambulatory Care Clinical Pharmacist at Kaweah Health Healthcare District, practicing in the Rural Health Clinics. She obtained her Doctor of Pharmacy from the Ohio State University College of Pharmacy and completed her PGY1 Pharmacy Practice Residency training at Salem Veteran's Affairs Medical Center in Salem, VA. She also completed a PGY2 Behavioral Care Pharmacy Residency at Community Health Network in Indianapolis, IN. Bradlee Rea, PharmD

Dr. Rea is a Clinical Pain Pharmacist at Kaweah Health. He practices in the outpatient setting at the Kaweah Health Specialty Clinic Center and helps by providing recommendations to practitioners who have patients dealing with chronic pain in some of the districts rural health clinics. Dr. Rea functions as the primary preceptor for the PGY-2 ambulatory care residency pain rotation (Pain I) and as a secondary preceptor to the advanced pain rotation (Pain II). He received his Doctor of Pharmacy from Southern Illinois University Edwardsville School of Pharmacy in 2015 followed by a PGY-1 residency at HSHS St. Elizabeth’s Hospital in 2016 and a PGY-2 Pain and Palliative Care Residency at Hospice of Southern Illinois in 2017. Here he practiced primarily in a community hospice organization with additional experiences in the treatment of outpatient chronic pain. Coleen Moriarity-Suggs, PharmD, BCPS

Coleen Moriarity-Suggs, PharmD is an Anemia Management Clinical Pharmacist at Kaweah Health. She received her doctor of pharmacy degree from the University of the Pacific School of Pharmacy. Dr. Moriarity-Suggs serves as an Assistant Clinical Professor for University of California San Francisco School of Pharmacy and precepts PGY-1 pharmacy residents as well as students from various schools of pharmacy. She was the recipient of the UCSF Apple for Excellence in Teaching Award for the UCSF Fresno site in 2013. Kelsea Ziegler, PharmD, BCACP

Dr. Ziegler is an Ambulatory Care Clinical Pharmacist at Kaweah Health. She practices at the Kaweah Health Specialty Clinic Center, and collaborates with other health care providers throughout the District. Dr. Ziegler serves as a preceptor for APPE students on Ambulatory Care rotation. She serves as a preceptor for the PGY-2 residency program. She received her Doctor of Pharmacy from Northeastern University in 2014. She completed her PGY-1 Pharmacy Residency and her PGY-2 Ambulatory Care Residency with Brookdale Hospital Medical Center in Brooklyn, NY in 2015 and 2016. Khushi Gill, PharmD

Dr. Gill is an Ambulatory Care Clinical Pharmacist at Kaweah Health and practices at Kaweah Health Specialty Clinic Center. She serves as a preceptor for APPE students on Ambulatory Care rotation and will serve as preceptor in training for PGY – 2 residency program. Dr. Gill received her Doctor of Pharmacy from University of California, San Diego in 2017. She completed her PGY–1 Pharmacy Residency and PGY – 2 Ambulatory Care residency with Kaweah Health.

Samantha Valle-Oseguera, PharmD, BCACP, BCGP

Dr. Valle-Oseguera is an assistant clinical professor in the Department of Clinical Pharmacy, based in the UCSF Fresno Pharmacy Education Program. Her clinical practice site is at Kaweah Health Medical Center in Visalia, California, where she serve as an ambulatory care clinical pharmacist at the family medicine clinic and primary care rural clinics. She serves as a preceptor for Kaweah Health's PGY2 Ambulatory Care residents, PGY1 residents on an ambulatory care rotation, and APPE pharmacy students from UCSF rotating through to the Fresno practice site. She graduated from the University of the Pacific in 2012 with her Doctor of Pharmacy degree. After that she completed a PGY1 and a PGY2 in Ambulatory Care with the Boise VA Medical Center.
