James McNulty, PharmD, Director of Pharmacy
James McNulty, PharmD is the Director of Pharmacy Services for Kaweah Health.
He has served in this capacity for over 10 years. He and his team have
been nationally recognized multiple times for pharmacy achievements, including
ASHP Best Practice Awards in 2010 and 2014. Dr. McNulty received his doctor
of pharmacy degree from Idaho State University in Pocatello, Idaho in
1997. He completed his PGY1 pharmacy residency at University of California

Rheta Silvas, PharmD, Assistant Director of Pharmacy
Rheta Sandoval, PharmD is the Assistant Director of Pharmacy for Kaweah
Health. She has been a Clinical Pharmacist for over 20 years, working
at Kaweah Health for over 15 years, serving first as the Inpatient Pharmacy
Manager for over 10 years before moving into her current role as Assistant
Director. She currently serves as a preceptor for PGY1 pharmacy residents
in the area of practice management. In collaboration with others on the
Pharmacy Management Team and staff, Dr. Sandoval was involved in the strategic
planning for and implementation Medication-related technologies including
Order Management Systems, Smart Pumps, Automated Dispensing Cabinets (ADC),
Anesthesia ADC's, Automated Parenteral Nutrition Compounder, IV Workflow
Technology, Bar Code Medication Administration, Automated Inventory Management
and Packaging Equipment. In collaboration with Department Leadership and
staff, she assisted in the establishment of Clinical Pharmacy Services
including Anemia Management, Antimicrobial Stewardship, Inpatient Anticoagulation,
Emergency Medicine, Pain Management, Medication Safety, and Pediatrics

Nicole Gann, PharmD, BCPS, Clinical Coordinator/PGY1 Residency Program Director
Medication Safety:
Dale Costantino, PharmD
Dale Costantino, PharmD, is the Medication Safety Coordinator for Kaweah Health. Prior to joining the Kaweah Health pharmacy management team in 2021, he served as a pharmacy director for 22 years. He has experience in directing pharmacy operations including clinical pharmacy services, pharmacy informatics, and regulatory compliance. As a Medication Safety Coordinator, he is responsible for working with other stakeholders, committees, and teams, to optimize the safe use of medications throughout the medication use system. He analyses internal data, literature, and regulations to lead error prevention strategies to prevent medication errors and subsequent patient harm. Formulary Management:
David Crowther, PharmD, BCPS, Formulary Management Specialist
David Crowther, PharmD, BCPS is the Formulary Management Specialist for
Kaweah Health and serves as a preceptor for both PGY1 pharmacy residents
as well as pharmacy students. He received his Bachelors of Science in
Physiology/Developmental Biology (2006) from Brigham Young University
and his Doctor of Pharmacy (2009) from Roseman University. Dr. Crowther
completed both his PGY1 Pharmacy Residency (2010) and PGY2 Drug Information
Residency (2011) at the University of Virginia Health System in Charlottesville
VA. After residency he worked as a Clinical Specialist in Drug Information
at MD Anderson Cancer Center in Houston TX for 6.5 years before joining

Kim Diep, PharmD, BCPS, BCGP
Kim Diep, PharmD, BCPS is a Clinical Pharmacist at Kaweah Health. She serves
as a preceptor for PGY1 pharmacy residents and pharmacy students from
the University of California, San Francisco and other pharmacy schools.
Dr. Diep completed her PGY1 pharmacy residency at Veterans Affairs Palo

Andrea Lewandowski, PharmD, BCPS is a Clinical Pharmacist at Kaweah Health.

Peter Weber, PharmD, MBA
Peter Weber, PharmD, MBA is a Clinical Pharmacist at Kaweah Health. He
serves as a preceptor for PGY1 pharmacy residents and pharmacy students
from the University of California, San Francisco. Dr. Weber received his
bachelors of science in biochemistry from the University of California,
Riverside in 2003, doctorate of pharmacy from the University of the Pacific
in 2009 and masters of business administration from the University of
the Pacific in 2010. He completed his PGY1 pharmacy practice residency
at St Joseph’s Medical Center in Stockton, CA. Dr. Weber has served

Bayode Fashemi, PharmD, BCPS, BCCCP, CGP
Bayode Fashemi, PharmD, BCPS is an Emergency Medicine Clinical Pharmacist

Dr. Hernandez is a Clinical Pharmacist primarily working in Internal Medicine

Chris Mahaffey, PharmD, BCPS
Chris Mahaffey, PharmD, BCPS is a Clinical Pharmacist at Kaweah Health
and is an Assistant Clinical Professor (WOS) at the University of California,
San Francisco School of Pharmacy. He serves as PGY-1 resident and pharmacy
student preceptor for Internal Medicine and Anticoagulation Management.

Chelsea Felix, PharmD, BCPS
Chelsea Felix, PharmD, BCPS is an Internal Medicine and Emergency Department
Support clinical pharmacist currently precepting pharmacy students and
PGY-1 pharmacy residents at Kaweah Health. Dr. Felix serves on the Residency
Recruitment Committee and was one of the pilot team members for the Transitions
of Care pharmacy practice model at the District. She completed her PGY-1

Patrick Binaday, PharmD, BCPS
Patrick Binaday, PharmD, BCPS is a clinical pharmacist at Kaweah Health
primarily practicing in Internal Medicine and Pediatrics/NICU. He is also
an Assistant Clinical Professor at the University of California, San Francisco
School of Pharmacy. He serves as a preceptor for pharmacy students as

Steven Richardson, PharmD, BCIDP, AAHIVP
Steven is a Clinical Pharmacist for Kaweah Health and a current preceptor
for the PGY-1 residency program. Steven has worked at Kaweah Health since
July 2018 and currently serves on the RAPID Preceptor Development and
Recruitment Committees. Steven completed his BS in Biochemistry at Presbyterian
College (SC), and his Doctor of Pharmacy degree at Texas Tech University
Health Sciences Center. After schooling Steven completed a PGY-1 Pharmacy
Practice residency and a PGY-2 Infectious Diseases residency at the Cleveland
Clinic in Cleveland, OH. His educational interests include cardiac hemodynamics,

Kellie Bartlett, PharmD
Dr. Bartlett is a Clinical Pharmacist at Kaweah Health and serves as an
Internal Medicine preceptor for both PGY1 pharmacy residents and pharmacy
students. She is also an Assistant Clinical Professor at the University
of California, San Francisco School of Pharmacy (UCSF). Dr. Bartlett completed

Nibal Fadhil, PharmD
Nibal Fadhil, PharmD is an Internal Medicine Clinical Pharmacist at Kaweah

Lawrence Chang, PharmD, BCPS is a Clinical Pharmacist at Kaweah Health.

Eva Coulson, PharmD, BCPS
Eva Coulson, PharmD, BCPS is a Pain Management Clinical Pharmacist at Kaweah
Health Healthcare District (Kaweah Health). She currently serves as a

teaching certificate, and precepts student pharmacists from the University
of California, San Francisco and other pharmacy schools. Dr. Coulson joined
the pharmacy team as a pain management specialist after completing her

Karly Erickson, PharmD, BCPS, BCCCP is an Emergency Medicine Clinical Pharmacist

Kelsey Zapata, PharmD, BCPS is an Emergency Medicine Clinical Pharmacist

Kelvin Tran, PharmD
Kelvin Tran, PharmD, is an infectious diseases specialist who serves as
the antimicrobial stewardship pharmacist at Kaweah Health. He serves as
a preceptor for PGY1 pharmacy practice residents who take an ID/antimicrobial
stewardship rotation. He received his Doctor of Pharmacy from the Ohio
State University College of Pharmacy in 2018. He completed both his PGY1
residency and PGY2 Infectious Diseases Pharmacy residency at Ochsner Medical
Center in New Orleans in 2020. His current clinical practice and research
interests include optimizing antimicrobial therapy and treating antibiotic

Coleen Moriarity-Suggs, PharmD is an Anemia Management Clinical Pharmacist

University of the Pacific School of Pharmacy. Dr. Moriarity-Suggs serves
as an Assistant Clinical Professor for University of California San Francisco
