GME Residency and Fellowship Verifications

Request a Verification

Residency and fellowship verifications requests may be submitted to the Graduate Medical Education Office: 

• Send an email to: llcarras@kaweahhealth.org 
  
  • Include the name of the trainee along with a Release of Information signed within the last 3 months
• Pay the fee if applicable 

Verification Fees

• Free- Standard Verification of GME training forms
• $50 - Non-Standard verification form, or your own institutional form
• $75 - Custom verification letter, request for hospital seal or notarization

There are no fees associated with: 

• Current or recent trainees who have graduated within the last academic year
• VA Medical Centers
• CA Medical Board

Payment Information

• Credit card only
  
  • Once request is received, our office will send the requestor a secure email requesting credit card information 
• Once payment is received, you will receive the verification within 5-10 business days by e-mail
