Outpatient Clinical Pharmacy Services

Pharmacy

Kaweah Health Outpatient Clinical Pharmacy services are supported by a dedicated team of eight clinical pharmacists, four pharmacy technicians and one PGY2 Ambulatory Care pharmacy resident. Our clinical pharmacists are credentialed through our medical staff office and work under a collaborative practice agreement. They partner closely with Kaweah Health providers to deliver personalized, expert care to patients.

  • Assisting with medication access and insurance coverage
  • Navigating formulary and billing challenges
  • Supporting administrative and care coordination efforts

Clinical Practice Sites 

Kaweah Health Tulare Clinic is the formal clinic training site for Kaweah Health Family Medicine Residency providing primary care for an underserved population. Our clinical pharmacists provide direct patient care, patient education, and practice-based and didactic instruction to the family medicine residents.

Kaweah Health Medical Clinic - Willow Specialty houses several pharmacy-led service lines, including Employee Wellness Programs, Medication Assistance Program and Pain Management Service.

Rural Health Clinics

Kaweah Health has six rural health clinics that provides primary care, women's health, pediatric care and specialty care services to rural areas of Tulare County. Pharmacists in these clinics are integrated into the primary care departments, where they see patients for a variety of chronic diseases and provide population-health management services.
