Temporary Change to Visitor Policy For Transitional care and Subacute Units

Visitation to patients in Subacute units and Transitional Care located at our South Campus (1633 S. Court Street) is allowed for 30 minute visits that must be scheduled in advance. Up to 2 visitors from the same household may visit per patient. Visits will take place either outdoors or in a large communal space such as a dining room or activity room, not in the patient room. All visitors must wear a mask and will be screened for symptoms upon entrance to the building. A staff member will supervise each visit to ensure proper mask use and social distancing. Visitation days are dependent upon the staff availability and may vary at each campus.

After your stay at Kaweah Health, you may need time to recover from treatment before returning home. Our transitional care unit is a 24-bed unit that supports short-stay patients in need of therapy services. This could include patients recovering from surgery, were admitted to the hospital for a long period, or need help adjusting to a new lifestyle.

Transitional Care Services emphasizes:

  • End-Of-Life care

Many patients transition to the TCS to prepare and gain strength to return home. A specialty area for TCS is our end-of-life/palliative care, also known as hospice care. Many patients and their families come to us to take their last journey at the end of life with palliative or hospice care. Patients and their families are provided education and support through the process by the nursing staff and other specially trained, caring staff members.

Family Involvement

At TCS, we encourage family involvement in caring for their loved one. Communication with the interdisciplinary team is frequent to keep the patient, family and all staff up to date with improvements and changes. Our patients are encouraged and assisted with acquiring habits that they would have at home such as getting dressed and performing their own hygiene routine (all with respect to the patient's ability). This is also done with the patient's goal in mind, which is often to go home soon and manage on their own if possible.
