At Kaweah Health Infusion Center, we value our partnerships with community providers. Together, we make it easier for patients to receive timely, safe, and comfortable outpatient infusion therapy close to home and supported by a compassionate care team.

How to Refer a Patient

To make your patient's referral smooth and efficient, please include the following information:

  • Completed order set
  • Authorization
  • Patient demographics
  • H&P or recent progress notes
  • Medication list
  • Labs as needed
  • Insurance cards (preferred, but not required)
  • Additional requirements as noted by insurance (i.e. Medicare requires IVPB form and documentation stating that patient cannot tolerate or failed oral iron and the reason why)

Next Steps

After we receive your referral, we will:

  • Verify benefits and authorizations
  • Assist with co-pay and financial resources as needed
  • Schedule and confirm infusion appointments directly with the patient
  • Update your office on approval status

Billing Info

Our place of service (POS) is outpatient on campus hospital (22).

We are a department of Kaweah Health and fall under Kaweah Health NPI and Tax ID. Please call us at (559) 624-2555 if you need these numbers to obtain authorization.

Our physical address is 325 South Willis St., Visalia. Our billing address is 400 W. Mineral King Ave., Visalia. Either of the addresses will work when obtaining authorization.

Billing and prior authorization info:

  • We bill insurance under the medical side and not the pharmacy side.
  • We cannot accept medication from outside pharmacies. There are specific exceptions to this rule, when we are working directly with the manufacture.
  • For Medicare patients we bill under part B and when there is a supplemental then the supplemental will pick up the 20% patient responsibility. We cannot bill under part D.
  • Medicare patients do not require authorization since we bill under part B
  • Straight Medi-Cal may require authorization please contact Medi-Cal or our office to verify if authorization is needed.
