PATIENT INFORMATION TO SHARE WITH EMERGENCY DEPARTMENT
Please call the Emergency Department at 559-624-2213 and tell them this information:
When was the last time the patient was awake and acting normally? Day and Time
Has the patient had any of the following?
- Head trauma or surgeries in the last 3 months
- Stomach, intestinal, urinary tract bleeding
- Blood thinner medication use
- Bleeding inside the head
- Previous stroke
- Brain aneurysm
- Bleeding disorder
- Liver disease
- Currently pregnant
Sharing this information will help your loved one get the medical care they need.

¿Cuándo fue la última vez que el paciente estuvo despierto y actuando de forma normal? Día y hora
¿Ha tenido el paciente alguno de los siguientes?
- Traumatismo en la cabeza o cirugías en los últimos 3 meses
- Sangrado en el estómago, intestinos o vías urinarias
- Uso de medicamentos para adelgazar la sangre (anticoagulantes)
- Sangrado dentro de la cabeza
- Derrame cerebral previo
- Aneurisma cerebral
- Trastorno de sangrado
- Enfermedad del hígado
- Actualmente embarazada
