Proof of Vaccine

Conditions are subject to change per California Department of Public Health.

Starting Wednesday, August 11th

All visitors must be fully vaccinated (two weeks from last dose of vaccine). OR Confirmed negative COVID test in the last 72 hours if unvaccinated or incompletely vaccinated.

Statewide measures are necessary to protect particularly vulnerable populations. Hospitals, skilled nursing facilities (SNFs), and intermediate care facilities (ICFs) serve uniquely vulnerable populations where COVID-19 outbreaks can have severe consequences including hospitalization, severe illness, and death.

California Department of Public Health Guidance for Vaccine Records allows the following methods to be used for proof of vaccination:

  • COVID-19 Vaccination Record Card with name of person, type of vaccine, and date of last dose administered issued by the Department of Health and Human Services Centers for Disease Control and Prevention or World Health Organization.
    • Photo, copy of card, digital record, or documentation from a health care provider is acceptable.
Unvaccinated Visitors Must Present Negative SARS-COVID-2 Test Collected Within The Last 72 Hours At Check-in For Each Visit

Only SARS-COVID-2 tests authorized by the U.S Food and Drug Administration or Laboratory requirements by the U.S Centers for Medicare and Medicaid Services will be allowed.

Exceptions to the state mandated requirement to provide proof of vaccine or negative test will include:

  • End of life or patient in critical condition when death may be imminent

  • Labor & Delivery

  • Pediatric/NICU population

  • Disability/Cognitive and Impaired patients

Patients' safety and well being is our top priority.

Regardless of vaccine status or test result - all visitors must wear a well-fitted facemask and adhere to Kaweah Health's visitor policy.
