Kaweah Health Rehabilitation Hospital offers two different settings, each designed to meet the specific needs of the patients served.

Acute Rehabilitation

• Short term, intensive program for adults whose medical and functional needs require a coordinated, multidisciplinary team approach.

• Typical diagnoses include stroke, spinal cord injury, amputation, major multiple trauma, brain injury, orthopedic conditions and neurological disorders

• Services provided by rehabilitation staff include:

  • Rehabilitation physician sees each patient daily, Monday through Friday. Physician coordinates care provided by the interdisciplinary team. Physicians available on call 24 hours a day, 7 days a week.

  • Nurses with specialized rehabilitation training and experience work with patients and families 24 hours a day, 7 days a week

  • Physical, Occupational and Speech therapy provided 3 hours per day, 5-6 days per week

  • Neuropsychology

  • Case management and social services

  • Registered dietician

  • Chaplain

  • Respiratory therapy

• Additional services such as orthotic and prosthetic fitting are provided as needed by referral

• Most patients are referred by their physician or case manager during their initial hospitalization. Referring hospitals include local facilities in Tulare, Kings, Kern and Fresno counties. Additionally, many patients return to Kaweah Health for acute rehabilitation after receiving specialty care in the Bay Area or Southern California.

• Patients receive information regarding program fees prior to admission. Patients and their families are informed throughout their stay of any coverage limitations imposed by their insurance company and alternative sources of funding are discussed as needed. Typically, coverage for the program is provided by commercial insurance programs, worker’s compensation, Medicare or Medi-Cal. Payer requirements such as severity of illness, functional needs, and rehabilitation potential are carefully assessed and patients informed of impact of those requirements on provision of services.

Short Stay Rehabilitation

• Provides a more flexible level of services for patients whose primary needs are to return quickly to independence in mobility and self care following orthopedic surgery or other interventions.

• Nursing care provided by nurses with specialized rehabilitation training and experience 24 hours per day, 7 days per week

• Physical and occupational therapy services provided 5-6 days per week, with intensity of services adjusted to each patient’s needs and goals.

• Physician supervision provided as appropriate to each person’s needs

• Typical diagnoses include hip and knee replacements, hip fractures, pelvic fractures, simple trauma, back surgery such as laminectomy and spinal fusion.
